3. Select the month in which you wish coverage to start. If you're applying online, click to submit the quote application.
4. Receive and review quotes.
Small Groups (50 or less employees)
5. The state of California allows small groups to receive quotes online, but other states limit the requests to mail, email or fax. Go to the Kaiser Quotes website to select a quote method available for your state and find the necessary contact information.
6. Provide the name, date of birth or age, zip code, employment status, and current health coverage information of each eligible employee.
7. Receive and review quotes.
Large Groups (51 or more employees)
8. Because large groups are subject to different rates, they must fill out a quote request package. Go online to the Kaiser Quotes website and indicate you are a large group. From here, you can download the package. Alternatively, you can request a meeting with an enrollment specialist who will provide quotes.
9. Provide information about your business' current health insurance plan as well as the name, date of birth, zip code, employment status, and current health coverage information of each eligible employee. Select the month in which you wish coverage to start.
10. Receive and review quotes.
